Feline Boarding Intake Form

BATHING: We recommend that all pets be bathed before discharge. If your cat is staying with us for more than 6 nights, a bath is required. A bath is optional for stays of 6 nights or less. Bathing is performed on Mondays and Wednesdays.

MEDICATIONS:

Any medications to be given to your pet or prescribed due to illness will be charged $4.75 per day to administer (excluding heartworm medications and vitamins).

ALL MEDICATIONS MUST BE IN THE PRESCRIPTION OR ORIGINAL BOTTLE

Medication

Please list any pre-existing conditions, hot spots, masses, sensitivities, allergies, injuries, or quirks your pet may have, so that our staff can be informed while they are here:
Please list all belongings brought in with your pet. Please be aware that toys may not be placed with your pet due to safety concerns, and rope toys are not permitted. While we take every precaution to prevent the loss, damage, or destruction of your pet’s belongings, we cannot guarantee their condition and replacements will not be offered. Any items left for more than fifteen days will not be returned or replaced.

Boarding Policies

VACCINATIONS:

All cats boarding at the hospital must be up to date on the following: Feline Distemper/Leukemia vaccination, Rabies vaccination, and a Fecal Analysis performed within the past 6 months. If proof of vaccination or testing is not available at the time of admission, the necessary vaccines and fecal analysis will be administered upon entry, and the client will be charged accordingly.

Cats are checked for fleas and ticks at the time of admission. If fleas or ticks are found, the cat will receive a flea bath, a dose of Capstar, and an application of Advantage Multi, all at an additional cost to the owner.

MEDICAL FEES:

Our goal is to keep your pet clean, healthy, and happy while they are boarding with us. If your pet becomes ill during their stay, we will provide appropriate treatment, and you will be responsible for all medical costs incurred. Before any major medical procedures, we will make every effort to contact you using the phone number provided. If we are unable to reach you, we will act in the best interest of your pet.

Occasionally, pets may develop issues due to environmental or dietary changes. Signs may include vomiting, diarrhea, coughing, sneezing, or self-trauma such as scratching or biting at the skin. We take great care to prevent these issues, and if they do occur, we treat them promptly. However, please be aware that these conditions can develop during or shortly after boarding, and the hospital is not financially responsible for these inherent risks.

CHECK OUT TIMES:

  • If your cat is not receiving a bath on the day of departure, check-out time is 11:00 AM.
  • If your cat is receiving a bath on the day of departure, check-out time is between 3:00 PM and 5:00 PM.
  • Additional charges will apply for late pick-ups.
