Canine Boarding Intake Form

ACCOMMODATIONS:

Boarding includes lodging in our specially designed kennels for your pet’s comfort, along with feedings using Science Diet Sensitive Stomach or owner-provided food. Fresh water is available at all times. Your pet’s quarters will be cleaned and sanitized daily.

Unsupervised outdoor exercise is provided twice a day in our securely fenced play yards. The yards are enclosed with a six-foot wooden privacy fence for your pet’s safety. On weekends and holidays, pets are cared for twice a day by our staff. Please note that pets are unsupervised overnight, as no staff members are on-site after hours. However, your dog is our top priority when staff arrives each morning.

Additional activity packages are available for your dog. Please note that all packages are subject to availability and weather conditions.

MEDICATIONS:

Any medications to be given to your pet or prescribed due to illness will be charged $4.75 per day to administer (excluding heartworm medications and vitamins).

Medication

Please list all belongings brought in with your pet. Please be aware that toys may not be placed with your pet due to safety concerns, and rope toys are not permitted. While we take every precaution to prevent the loss, damage, or destruction of your pet’s belongings, we cannot guarantee their condition and replacements will not be offered. Any items left for more than fifteen days will not be returned or replaced.

MEALS
We feed Science Diet Sensitive Stomach dry food. If you would like to provide food, please prepare it in meal-sized bags (one per feeding).

Please list any pre-existing conditions, hot spots, masses, sensitivities, allergies, injuries, or quirks your pet may have, so that our staff can be informed while they are here.

Boarding Policies

VACCINATIONS: All dogs must be up to date on their vaccinations. It is the owner's responsibility to ensure that proof of current vaccinations, testing, and heartworm prevention is on file at the time of admission. If proof is not available upon admission, vaccinations, testing, deworming, and/or heartworm prevention will be updated at that time, and the client will be charged accordingly.

Dogs must be current through their departure date for the following: Distemper, Bordetella (within 6 months), Heartworm Test, Rabies, and a fecal exam and/or deworming (within 6 months).

All dogs are checked for fleas and ticks upon admission. If fleas are found, the dog will be given a flea bath, Comfortis will be administered, and Advantix will be applied—all at an additional cost to the owner. If ticks are found, a tick bath and Advantix will be required, also at an additional cost to the owner.

MEDICAL FEES: Our goal is to keep your pet clean, healthy, and happy while they are boarding with us. If your pet becomes ill during their stay, we will provide the necessary treatment, and you will be responsible for all medical costs incurred. For any major medical care, we will make every effort to contact you using the phone number provided. If we are unable to reach you, we will act in the best interest of your pet.

INHERENT CONDITIONS: Occasionally, pets may develop issues due to environmental or dietary changes. Symptoms can include vomiting, diarrhea, coughing, sneezing, or self-trauma such as excessive scratching or biting of the skin. While we take every precaution to prevent these issues and treat them promptly if they arise, please be aware that such conditions can develop during or shortly after boarding. The hospital is not financially responsible for these naturally occurring conditions, should they occur.

ABANDONMENT: Please notify us of any changes to your pet’s scheduled departure date. If we do not hear from you and are unable to reach you for a period of 10 days beyond the scheduled departure date, the hospital will consider your pet abandoned and will proceed in accordance with North Carolina pet abandonment laws.

CHECK OUT TIMES:

  • If your dog is NOT enrolled in all-day doggie daycare or scheduled for a bath on the day of departure, check-out time is 11:00 AM.
  • If your dog is enrolled in all-day doggie daycare or receiving a bath on the day of departure, check-out time is between 3:00 PM and 5:00 PM.
  • Please note that additional charges will apply for late pick-up.
